• Registered Nurse (RN): Our RNs possess extensive medical knowledge and clinical skills. They perform patient assessments, administer medications, manage complex care needs, educate patients and families, and collaborate with doctors to ensure a comprehensive care plan.

• Physical Therapist (PT): Our PTs create personalized exercise programs designed to improve mobility, strength, balance, and coordination. They help patients recover from injuries or surgeries, manage chronic conditions, and regain functional independence for daily activities.

• Occupational Therapist (OT): Our OTs help patients regain or improve the skills they need for everyday living activities such as bathing, dressing, preparing meals, and managing medication. They can also recommend assistive devices and modifications to the home environment to promote safety and independence.

• Speech-Language Pathologist (SLP): Our SLPs evaluate and treat communication difficulties caused by stroke, brain injury, or other conditions. They can help patients improve their ability to speak, understand language, swallow safely, and use assistive communication devices.

• Licensed Medical Social Worker (LMSW): Our LMSWs provide emotional and social support to patients and their families. They can help patients cope with the challenges of illness, connect with community resources, and navigate the healthcare system.

• Licensed Vocational Nurse (LVN): Our LVNs work under the supervision of RNs to provide basic care services such as taking vital signs, assisting with bathing and dressing, and monitoring medication administration.

• Home Health Aides: Our compassionate home health aides provide assistance with daily living activities such as bathing, dressing, grooming, meal preparation, and light housekeeping. They offer companionship and emotional support, helping patients maintain a sense of well-being at home.



• Primary Home Care Providers: We collaborate with your primary care physician or other healthcare providers to develop a comprehensive care plan that aligns with your specific needs and medical directives.
